1) Your full name at birth and any other names you have had in this lifetime.
2) Date of Birth
3) Names of other people living in your home and the name of other significant people in your life (parent, friend, relative, lover, etc.) and their relationship with you.
4) A phone number to reach you at to schedule a conference time.
5) Photo of yourself. This can be e-mailed to lisareber@your-souls-journey.com
6) A handwriting sample. This can be done with your name(s) and date of birth. This information can be faxed to Lisa at 920-921-5572 or scanned and e-mailed as an attached file.
